Please fill out and submit this form to request flowers be placed in the sanctuary in memory or in honor of your loved one(s). Our office will be contacting you to confirm the information. You will be responsible for ordering the flowers. You will need to contact Audrey Clance for information about ordering and local florist. The florist must make arrangements in advance to meet with someone on Saturday for delivery.
